Discontinue MDVIP Relationship
After much careful consideration, we have decided to discontinue our relationship with MDVIP, effective December 6, 2004. However, we will be continuing our personalized medicine practice independent of MDVIP, and beginning December 7, 2004, we will be accepting patients directly into our Personal Docs program.

Our departure from MDVIP will not affect your health care. We will continue to honor your current MDVIP contract until it expires. At that time, you will have the option of choosing a new doctor in the MDVIP system or continuing with us in our personalized practice. We will continue to provide you the same level of care you expect and deserve from us. Under our new program, as before, we will be providing the following services for an annual fee of $1,500.00 per person, payable quarterly, semi-annually or in fall.

As of December 7, 2004 the services remaining the SAME are:

1. An annual comprehensive physical and wellness plan including laboratory studies, EKG and screening chest x-ray

2. A wellness follow-up visit to review health maintenance recommendations and facilitate scheduling of necessary tests or specialist's appointments

3. Periodic newsletter and evening speakers programs

4. Benefits of a smaller practice which include:

On-time appointments, same day urgent visits, assistance with scheduling appointments and referrals, facilitation of prescriptions by phone, physician access by pager 24 hours a day, email access to physicians, waiting room amenities, and thorough follow-up of test results and specialist's recommendations.

Services that will be DIFFERENT are:

1. Payment for the annual membership fee will be payable to "Personal Docs" and will be sent directly to our office. Until your renewal time arrives, please continue to send your payments to MDVIP. Again, we will continue to take care of you through MDVIP until your contract with MDVIP is up for renewal.

For our patients whose MDVIP annual renewal time is December and who would like to stay with us, enclosed is our new Personal Docs annual agreement. For those of you whose MDVIP contracts expire at different times of the year and who would like to stay with us, you will receive a Personal Docs agreement from us at the appropriate time.
